Applying for Accreditation Certificate

    1. Fill Out the Form

    Go to the ugm.id/FormLegalisirdanSertifikat and complete the required form.

    Please prepare Diploma Certificate and the Request for Accreditation Certificate! (click here)

    1. Confirm via WhatsApp

    Contact the Academic Alumni Office of the Faculty of Agriculture UGM (08112955067) via WhatsApp to confirm your submission.

    1. Processing the Application

    Your application will be processed by the Academic Alumni Office of the Faculty of Agriculture UGM.

    1. Administrative Fee Information

    The details of the administrative fee will be communicated to you via WhatsApp.

    1. Send Proof of Payment

    After making the payment, send the proof of transfer to the same WhatsApp contact.

    1. Receive the Accreditation Certificate

    A copy of the accreditation certificate will be sent to you.

    1. Complete the Service Evaluation Form

    Fill out the academic service evaluation form at ugm.id/KuesionerAkademik.
